What is hospital discharge care?
Hospital discharge care is a service when the live-in carer is at the Customer’s disposal immediately after they are fully recovered and discharged from the hospital, or even earlier.
If needed, it is possible that the carer is already available before the Customer leaves the hospital. The carer can prepare the Customer’s home for their return, so the hospital-home transition is smoother.

It is extremely important for us that our Customers and their families receive all the support needed, at every possible stage of care.
Admission to hospital, whether planned or not, is always a worrying and hard time, particularly for the older person. At times members of the family of a person that leaves the hospital are unable to provide their loved ones with the care immediately after the treatment is ended and it sometimes may delay the discharge. That can be an additional burden to the hospital, as beds are simply blocked by patients who really would prefer to come back home where they would most probably recover more quickly. Patients in private hospitals may end up staying in the facility longer than required or can afford. It also causes unnecessary stress for the family who cannot be with their elderly relatives at the time of hospital discharge. It is also disturbing for the patient himself. That’s why we are offering help in organising live-in carer as soon as it’s necessary.
Arranging care before the patient leaves the hospital
It is crucial that before a person is discharged from the hospital, their care needs are assessed so that any care services or support they may need is arranged before the person leaves the hospital. In case a live-in care service is required, this upfront information enables the caregiver to find the carer timely.
